You must print and complete the official medic form before booking an appointment. This must be filled in and signed by a registered medical professional (medic/nurse/pharmacist).
The completed form is required to complete an appointment booking.
It is now mandatory for first-time offshore workers and crane operators to complete an eyesight test.
In the UK, opticians will carry out a free eyesight assessment, including checks that can identify signs of serious disease. Regular eyesight checks with a qualified optometrist are recommended.

Once completed, upload the form to the 'Upload Eyesight Test' section in your account before your medical appointment.
Audiometric screening is also provided by audiologists including Specsavers at no charge: please request a copy of the audiogram and upload it to "My Files".
